- Be informed of rights and responsibilities
- Have a family member chosen representative and/or his or get own physician notified of admission to the Behavioral Health Provider (BHP) at the request of the client.
- Receive treatment and medical services without discrimination based on race, age, religion, national origin, gender, sexual orientation or disability
- Maintain the personal dignity of each client
- Be free from abuse, neglect, exploitation and harassment
- Receive care in a safe setting
- Receive services of a translator or interpreter, if applicable, to facilitate communication between the client and the staff
- Be informed of the client’s own health status and to participate in the development, implementation and updating of the client’s treatment
- To make informed decisions regarding the client’s care by the client or the client’s parent or guardian, if applicable, in accordance with federal and state laws and regulations
- Participate or refuse to participate in experimental research when the client gives informed, written consent to such participation, or when a client’s parent or legal guardian provides such consent, when applicable, in accordance with federal and state laws and regulations
- Be informed in writing of the policies and procedures for filing a grievance and their review and resolution
- Submit complaints or grievances without fear of reprisals
Client Responsibilities
- Provide complete information about one’s illness/problem, to enable proper evaluation and treatment
- Ask questions to ensure an understanding of the condition/s or problem
- Show respect to health personnel and other patients
- Reschedule/cancel an appointment so another person may see a physician and/or specialist
- You and your family are responsible for following the treatment plan developed. Any concerns. Any concerns you have should be expressed to your clinician.
- You and your family are responsible for the outcomes if you do mot follow the treatment plan.
